718862 - Add a new step 5 (after sceenshots)

After a person adds screenshots we need to add another step about payments.  From the spec:

> As part of submission, developers will indicate whether their app/add-on is:
>> entirely free
>> free, but requesting contributions (tips)
>> free, using in-app purchases (whether our payment provider or theirs)
>> paid, optionally using in-app purchases too
>
> The page that asks this should have brief information about each option with links to 
> learn more about our payment options and documentation.

The "brief information" is available halfway down in the spec[1].

Each option has a flow that should appear on the page, making this a FatBug(TM).  Also, mockups are incoming.
