Greg showed me a preliminary version of this, and I think it's hot. A PSD/PNG will suffice and we can close this and open an implementation bug.

I don't have the Manage PayPal/Payments page stuff started (I'm going to work on it today), but I do have a new layout for the Payments step in the create app flow.

It is confusing to me that you can't select 'free app' then select an in-app payments box. The app is indeed free in this case.
