The JavaScript loader appears when the hangout is loading, and the notification icon for click-to-play plugins is shown in the location bar, but enabling plugins manually happens too late for the page to allow it to work.

The page then redirects to "Install the Google+ plugin" or some text like it.
Still a problem on Firefox 17.0.1.
There's lots of C2P fixes in Firefox 18-20, would be great if someone could try to repro this in a beta, aurora, or nightly build. Thanks !
This still occurs in nightly. The plugin does get spawned properly but the site does not retry after the initial failure.

Bug 819555 would help with this -- the site does prompt the user to try reloading -- but otherwise this is likely an evangelism bug

I have set plugins.click_to_play to true.

If I start a google hangout, for 2 seconds the small popup appears where I can activate plugins. Then it disappears. To fast to activate.

Also, the bluee box to the left of the URL bar disappears.

So it is not possible to create google hangouts.

With flash-pages, this works fine.
I'm definitely interested in the case where the plugin icon disappears.

I tried to write a small testcase for this behavior. It loads a plugin, and then removes it after 2 seconds:

In this case, the blue plugin icon stays around as long as the page is loaded, which I believe is the desired behavior. This gives (or will give) users the option to "always for this site" or "reload with plugins enabled".

I wonder in what way G+ is different from this testcase. Perhaps it is loading the plugin in a frame and then unloading the frame? Would anyone be willing to debug the sites and/or write some testcases to figure out what's going on?

I downloaded the pages that Google uses for hangouts, including the one where it doesn't work.   Offline, all of them gave me the option to enable plugins.

Worth mentioning, when offline the two pages for loading a hangout, only prompt me for Google Talk plugin.  Whereas the main hangout page prompts me also for the Video Accelerator.

I think your test case does actually show an issue seen online.  Once the redirect happens and you can still click enable.. you then have to refresh the page, which seems to reset that you enabled the plugin.  And you start again.

This is currently by-design - "activate"/"activate all plugins" only makes them active until reloading or navigating.
"Always activate plugins for this site" should work however and UI work is still ongoing (e.g. "always activate" is supposed to be more promiment).
I believe this is working now on trunk/aurora. Could somebody please verify?
