Steps to reproduce:
1. Load details page for an app (ex. https://marketplace-dev.allizom.org/app/test-app-smurfs/ )
2. Click the Report Abuse button.
3. Observe the Header from the Report Abuse page.
Expected results:
Header title is displayed.
Actual results:
Header title is missing.
Notes/Issues:
Verified in FF18 (Win 7).
