790142 - Send email notification when a reviewer reaches a new point level

We are sending gear to reviewers as they accumulate points for reviewing add-ons and apps (themes will come later).

Here is the table indicating corresponding levels and associated gear:

Level 1     2,160 points     Stickers + Thank You Note
Level 2     4,320 points     USB drive
Level 3     7,200 points     Shopping bag and water bottle
Level 4     13,500 points     Marketplace Reviewer t-shirt
Level 5     24,000 points     Marketplace Reviewer hoodie
Level 6     96,000 points     Tablet

When a reviewer accumulates 2,160, for example, we should send an email notification to [e-mail], and subsequently for every new level reached.

The email should contain the following information:

Subject line: An add-on/app/theme reviewer has earned gear!
Username of reviewer
Email of reviewer
Level reached
Number of points

The subject line should drop in the type of reviewer automatically.

Closing this out; we run a weekly report so probably don't need an email notification as well.
