The existing bugzilla gear request form [1] needs to be updated to include new information about ordering and required fields from our partner, Staples. 

I have attached a pdf with updated information. Also, can the auto-cc's be updated to jbalaco@mozilla.com and gearteam@mozilla.com.

Changes to the component Swag Requests have been saved:
  Default CC list updated to gearteam@mozilla.com, jbalaco@mozilla.com
This looks great. I have a couple of additional updates, if possible. 

1. Can My MozSpace be a dropdown with the following options: 
Mountain View, San Francisco, Toronto, Vancouver, London, Paris, Berlin, n/a

2. Can the Team + Department also become a dropdown? There are a little more than 50 department codes that would go in that list. What's the best way to send this list along? 

3. Can #4 and #5 be combined to read: "If approved, your order will be sent to Staples for processing and shipment. If your order is not approved, you will receive an explanation why it was not approved or a request for more information.

4. Remove "You will be notified by email of any progress made in resolving your request." at the bottom.

attach the list as a text file to this bug would be the simplest.
to be used for the team + department code dropdown

Kate, you'll need to create a new Bugzilla account on that staging site if your Bugzilla account was created after September 2011. The current database is a snapshot of production from September 13, 2011.
due to email being disabled you can't self-signup on bugzilla-stage-tip.
i've created an account for kate and emailed the details to her.

This is looking awesome! I have a few more edits to make to the page:

Top section: 
1. Add "We do this to help protect the personal identifying information in this bug." after "Requests will only be visible to..."

2. Add a field for "Link to Mozillian profile" after email address

3. Add "(if applicable)" behind "My MozSpace" and don't make it a required field

4. If someone selects "Mozillian Recognition" as the Purpose of Gear, can it pop-up two follow-up question (with a check-box for yes/no answers) that say "Does this bug need recipient shipping information?" and "Does this bug need recipient size information?" 
 
When the answer to the first question is yes, is there a way for the bug to make the address information no longer required? The First and Last Name plus recipients email would still be required. 

4. Add "Date gear is needed by" under "Purpose of gear"

5. Add "Email" after Last Name in the Ship to Information section. 

6. Add "include country code if ordering from outside of the US" behind "telephone"

-Change form name: Mozilla Gear

-Remove current text at the top and add:

* Requests are reviewed every Monday. If approved, we'll let you know and then send your order to our gear partner, Staples, for processing and shipment; if it can't be approved,  we'll email you with details (or possibly ask for more information).
   
-Edit: “Requests will only be visible...” to
“Requests will only be visible to the person who submitted it, authorized members of the Mozilla Engagement team, and our Staples Customer Service rep. We do this to help protect the personal identifying information in this bug.  
-And move to the bottom of the page to replace “If this is a time-sensitive...”

-Move "Purpose of Gear" section above contact information.
-Add heading: Tell Us What You Want
-Add: Gear Options: View the current inventory at https://wiki.mozilla.org/GearStore/Gearavailable
-When selecting Mozillian Recognition => shipping information needed, Shipping First, Last + Email need to remain required fields.

you'd probably want to rephrase that sentence, so i've left it out from the latest revision.  all other changes have been made.
That looks great - re: add current inventory link and the rest of the changes. 

For the note to be added under Date Required:
If this order is time sensitive, place a Space Request [link to https://wiki.mozilla.org/GearStore/SpaceRequests] to pick up your gear from your Mozilla Space.
gah, i just realised that i made the changes on the 16th, but forgot to let you know that bugzilla-stage-tip has been updated.  sorry about that.
No problem. We actually saw that it was updated and have had a few other people take a look to give us feedback. I will have some final changes this afternoon (after I meet with Mary and Kate to review the feedback).
I was wondering if it might be possible to incorporate the gear options into the form, so that they auto-populate the information (item number, description, value) that we need to process the requests.   

The list of gear is at https://wiki.mozilla.org/GearStore/Gearavailable. 

There are actually less than 20 items, but it gets tricky with the items that have sizes. Each size has a separate item number. Also, the t-shirts come in unisex and ladies styles and 3 colors. Do you think this is something that could be added relatively easily? How much time do you think it might take?

i don't think it would be that tricky to achieve. probably a couple of hours work.

Wow! This is fantastic. Hopefully this is the last list of requests:

1. Can the price removed? 

2. Can the item # be added? I attached a text file with the list of item #s and item descriptions. 

3. Is it possible to have a quantity box show up next to an added item so that a person placing a request for multiple of the same item wouldn't have to add it over and over? 

4. If a Mozilla Space is selected, can a check box show up that says "Ship to this Address". And if the box is checked, can the shipping address auto-populate with the correct office address. I have also added a text file with the office addresses - it's missing Berlin but I will add it here as soon as I get it.

5. I'm not sure what the exact URL for the form will be, but if it includes the word "swag", can that be changed to "gear"?

the item number's already there, but only on the generated comment.
eg. https://bugzilla-stage-tip.mozilla.org/show_bug.cgi?id=686855

when i had the item number in the select, it made it much harder to read.

i'll make both work, so existing links don't break.


there's no problems with your other requests, will work on those at some stage today.

Thanks! That makes total sense on the item number. 

I have a few (hopefully) final changes:
 
1. The bullet point "Follow the steps below..." needs a parenthesis after "instead".
2. Same bullet point - change "an emergency" to "a last-minute request"
3. Under purpose of gear - change "Gear Closet at a MozSpace" to "Gear Space Stock"
4. Remove colons in the Purpose of Gear dropdown 
5. Under Tell Us Where to Ship, add note: 
   "Please be aware that shipping can cost as much as, if not more than, your item. And, items shipped internationally incur customs fees that can be 100%+ the cost of the package. When possible, requests will be filled from gear at Mozilla Spaces." 
6. Remove "Shipping" from the fields in the Tell Us Where to Send It section.
7. Change "Shipping Telephone" to "Recipient Telephone"
8. Remove hyphen at the bottom between "*" and "Required Field"

Thanks again for helping us get this form to the way it is now. It looks great!

I just wanted to check on the status of these changes:

A member of the engagement team tried to submit two requests today and received the following error message: 

"You tried to restrict a bug to the 'mozilla-engagement' group, but either this group does not exist, or you are not allowed to restrict bugs to this group in the 'Marketing' product."

Do you have any idea what that might be related to? I was able to successfully place a request last week. Thanks for looking into this. Let me know if you need any additional information from me.

The group configuration looks proper on the upgraded system as well as the same three people are members of mozilla-engagement-team (jbalaco@mozilla.com, knaszradi@mozilla.com, mary@mozilla.com).

Who was the person who was trying to create the new gear request and was it someone not on the above list? We could make the group "always fileable" which means anyone file a new gear request but they will only be able to see *their* requests.

Jennifer Bertsch was trying to place a request, and she is not one of those three people. 

Please make the group "always fileable" because any Mozillian should be able to place a request. Will that change be live immediately or will it take a weekly build to be made?
