Sure, I can do the icon, but we should clarify behavior and placement.  For notifications to be more effective, surely users could see them even when not using Firefox.  This suggests using global, OS-level notifications rather than just an icon in Firefox.  In OSX, that would be the top Apple Menu, and in Windows that would be the notification tray.  Not sure about Linux.  This bug is opened for Linux, but I'm assuming we need all three platforms.  Could Shane or another dev (I believe he's traveling at the moment) clarify?

I think this bug is asking for the _permission prompt's_ icon, not the actual notification itself.

Attachment 763893 [details] shows the last version of the prompt, which is conspicuously lacking an icon.

(Also note that bug 782211 added chrome://browser/skin/notification-64.png, but Push Notifications involve a rather different form of permission... See bug 868322 comment 60 and 62.)
