Steps to reproduce problem:
1. Visit page with CTP plugin
2. Page removes plugin element
3. Click doorhanger
Attachment 770720 [details] works if you have Java installed, but you can easily tweak it to use a different plugin.

PluginRemoved was added by bug 889788. This patch avoids the bug 916542 issue by explicitly looking for a click to play plugin, which means that we also handle the edge case of removing the last such while playing plugins are still present.

Firefox Bug 889788 landed on mozilla25. We need to land this patch on aurora and beta.
