We have a current report template that should be adjusted to contain the fields collected by our new report submission form (bug 928444).

Specific specs:

Reports are publicly viewable. Admins and the Rep who created the report will see an 'Edit report' button. The Rep who created the report sees a string "that's you" under her name. Any signed-in user can add a comment to a report.

Attached a static mockup of the existing template with updated form fields and visual styling.

Still a few things to tidy up, but unless there are any changes required based on feedback here this one is pretty much ready.

This looks great, Alex! One small nitpick is the date format is slightly different on this view report mockup (YYYY/MM/DD) than the create report mockup (YYYY-MM-DD). I suggest updating the date form on the view report mock to be YYYY-MM-DD, so it is consistent on both pages.
