The block is now staged with ID g463. I set the OS to WINNT 5.1, which I assume is what you intended.

I have verified this on staging using Fx26 using the the instructions in https://wiki.mozilla.org/Blocklisting/Testing#Testing_Graphics_Blocklist_entries The g463 is added in the blocklist. I don't have a hardware XP machine, so I cannot test this end-to-end, but I think this should be sufficient.

Yes, this was tested. We can confirm that 6.14.10.5218 fixes the problem. There are multiple discontinuous ranges of drivers where it's broken, but we have had success with version 5218 and higher that we could get our hands on.

OK, now tested for real, following the instructions from comment 5. Magical. It works, the driver version is blocked.
