Details

We talked about this being a Google Form or Bugzilla Form.  Either option should make it clear that a contract is necessary and the form should only be allowed under very specific circumstances.

For the sake of simplicity I vote Google Form. :)  Let me know if I can help with this.

Based on our discussions today, we're wontfixing this.  We need to decide if we're doing prefs or different CA roots.  This won't block our first launch so closing the bug.
