We should use the beautiful new form design we created for the EOY 1-page donation form. This doc outlines the specific elements I think we can transfer from that form to our new advocacy petition form. Also included is a rough mockup of the "thank you - email signup" page, also adapted from our post-donation signup form. Please let me know if you have Qs.

mavis have you created the page name in BSD yet? I want to make sure we're using a solid naming convention (sometimes our names are too general and it makes it difficult to do follow-up audience queries with granularity). I would name this page something like: "20150226 FCC Vote Net Neutrality Petition"
