Since posting earier today, I launched in safe mode and all is OK. Relaunched in normal mode and issue was back. I then refreshed Firefox completely which deleted all Add ons. I re-installed the LastPass addon and the issue was back instantly. I've now just installed Lastpass after a few other vital addons and all seems to be OK. So, this is turning out to be weird!

I am now seeing the same issue. I don't use any plugins to my knowledge though...
I'm having the exact same issue:

Creating a new profile fixes the issue, but disabling every add-on in my existing profile seems to make no difference.

Is this from the affected or the unaffected profile? Can you compare the Graphics sections of about:support of the two Firefox profiles?
System: Macbook Pro 15" Retina (OS X 10.10.3), hooked up to an external Dell P2715Q monitor (a 4k monitor @ 3840x2160). It is set to the middle scaled option, aka "Looks like 2560x1440."

If I shrink the window (to something like .75 * max_width), the black bar will disappear. I can then re-enlarge it and the black bar stays away. However, if I quit Fx and restart, it will reappear.

FYI, I'm pretty much always on IRC (as April), and would happily do a Vidyo (or whatever) chat to demonstrate.

I think the only reason why the unaffected profile isn't having the issue is because the browser doesn't automatically started maximized.
I have been experiencing this same issue. I am running v44 and am connected to a 4k Dell P2415Q.

This at least partially seems to the state FireFox was in when it was quit the time before launching.

Steps I can reproduce the issue.

Prep (when the bar shows up is dependent on how things are prepped):
- Maximize FF on my laptops internal screen.
- Quit FF

1) Start FF on my internal screen. It starts maximized with no black bar.
2) Put FF into full screen mode. No black bar
3) Move full screen FF to external monitor. No black bar.
4) Quit FF
5) Attempt to launch on external monitor but FF starts maximized back on my internal one - still no black bar.
6) Move FF to external monitor
7) Maximize FF
8) Quit FF
9) Attempt to restart FF on external monitor. I see a tiny little app popup for half a second before FF seems to open anyways on my internal monitor.
10) Move FF to external monitor.
11) Put FF into full screen mode
12) Take FF out of full screen mode (maximized) - still no black bar.
13) Quit FF
14) Start FF on external monitor - this time it takes and FF starts maximized on my external monitor and the black bar is present.
I am experiencing the same problem as well with a brand new 4K Dell P2715Q which is what Matt and I seem to have in common. I am running at 3008 x 1692 res on my new monitor.

Black bar across tabs. This black bar goes away if I resize my FF window to about 50% of it's original size. I can then resize the FF window back to nearly full screen without the bar appearing. When I close and re-open FF, the black bar re-appears.

I am on FF 47.0.1, OSX 10.11.5. Disabling my 2 addons makes no difference, and I am not running any Themes.

If I turn off Hardware Acceleration in Preferences, I get a completely black window when I start Firefox. I have to restart in Safe Mode to get rid of this black window and re-enable hardware acceleration (because I can't see the preferences).
Can you check whether this issue is fixed in this build of Firefox? http://archive.mozilla.org/pub/firefox/try-builds/mtseng@mozilla.com-73cc805fc4456715763a78b5219e89a2a932c25d/try-macosx64/firefox-50.0a1.en-US.mac.dmg

This is a build that contains the patch from bug 1281686.
Unfortunately that link provides a Nightly build that opens up the DMG fine, but the actual application file "is damaged and can't be opened". I've tried downloading it twice with same results - possibly the build is damaged?
That's strange, the build works on my machine. (I'm on 10.11.6 Beta, but that shouldn't make a difference, hopefully.)
I downloaded it again from this location:

http://archive.mozilla.org/pub/firefox/nightly/2016/06/2016-06-28-03-02-38-mozilla-central/

and I was able to install and run it properly (the app in that download was called FirefoxNightly and not just Nightly).

However, the same problem exists, and doing the resize trick fixes it as well (until next launch at least).
Thanks for testing!
Turns out my fix posted above (and what Matt posted as well) falls apart when you go to a large Google Maps browser window. To replicate click this link:

https://www.google.com/maps/@33.9947833,-118.4474032,192a,20y,305.44h,47.2t/data=!3m1!1e3!5m1!1e4

If I click on that link, I get an entirely black screen except for the Google maps labels hovering in dark grey above it.

If I try the resize trick described above, the map will become visible like I'm used to seeing when the window is small. However when I resize it, it will do an odd vignette effect when I make it bigger again, as per the attachment I added to this case.
Vignette effect when resizing Google Maps browser windows.
(FWIW, I can't reproduce this. I also tried on my P2715Q.)
Component: General → Widget: Cocoa
Product: Firefox → Core
Reproducible, Firefox 49.0a2 (2016-07-05) on MacOS 10.11.5 with two Dell P2715Q (scaled to 2560x1440). Resizing the window (with black bar) to a smaller size makes the black bar go away. Resizing to the original size keeps that state. Restarting Firefox brings the black bar back again.

I'm having the exact same issue in my brand new LG 27UD88W (2560x1440), I can confirm if I size the window down and then put it back as it was the black bar disappears, I am running Firefox 47.0.

I just updated to 47.0.1 and same thing.
Problem still exists with 48.0.1. Google Maps is unusable with my setup because of the image I added in this bug report in June. If you're having the black bar problem, can you try Google Maps in a very large window and see if it does the same thing?
With this bug apparently unfixable (or no traction to fix it), I'm going to have to stop using Firefox. While the black bar on the title is annoying, it's at least fixable by a quick resize.

Unfortunately, I see this "black screen mentioned" in my above Google Maps post more and more and my only solution is to switch to another browser every time I use a mapping program, which is frequent in my line of work. Anything from Google Maps is unusable unless you resize the FF screen down to what appears to be 1280 x 720. As soon as you resize it larger than that, everything in Google Maps turns black except for the labels.

Note that Bing Maps and Yahoo Maps work fine, sadly they are not tied into my Google ecosystem.

I'm going to upgrade to Sierra to see if that makes a difference, however I have my doubts. I'll check back in this thread periodically to see if the team has resolved the bug.

I was not able to reproduce this issue on Firefox Release 89.0.2 on macOS 10.15. Since the issue is no longer reproducible and Firefox has changed quite a bit, I'll close it as resolved WFM. Please feel free to reopen it if you are still experiencing the same.
