Reproducible on Firefox 38 beta 2 (20150406174117), latest Dev Edition 39.0a2 (20150407004006) and latest Nightly 40.0a1 (20150406030204) under Ubuntu 14.04 32-bit, Win 7 32-bit and Mac OS X 10.8.5.
Steps to reproduce:
1. Go to https://www.youtube.com/ and select a video (video 1)
2. While video plays, select another video from the page (video 2)
3. Add it to the reading list
4. Remove it from the reading list and add another video from the page
Expected results:
3. The second video (video 2) is added to the reading list
4. Video is added to the reading list
Actual results:
3.4. The name of the video played the first time (video 1) is added to the reading list
In both cases, the url is correctly saved, the preview thumbnail and the title are wrong.

I can't reproduce this on OS X -- can you expand on what's happening in step 2? When you're "selecting another video", is the URL being replaced to play it? Not sure what's happening here.
I almost wonder, though, if this is some variant of YouTube using history pushState to update the page without us noticing? </wild guess>
Preemptive P3, but mostly because it seems not so useful/likely that people will add YouTube to the reading list.

Youtube changes the url in location bar when another video is played.
Please see below youtube links so that there is no misunderstanding:
1. Search "60fps" in youtube
2. Open https://www.youtube.com/watch?v=lfwjzNB--5k (The Avengers), play video - DO NOT add it to Reading List
3. Select another video from Youtube's right side https://www.youtube.com/watch?v=wE4t9iBq_9E (Star Wars was the first one for me) - ADD IT to Reading List:
=> The Avengers thumbnail and title is added to the Reading List.
When it is clicked, Star Wars video plays.
Going further and trying to add other videos, all displayed on the same page (Youtube suggestions) to Reading List, bug 1151871 happens, but I logged them separately to highlight the thumbnail-summary issue.
