Seth has seen this, too.  Sorry, Peter, I don't know to whom this should go:
whether it be session history, or DOM, etc (which would be outside your group).

Summary: Occasionally, the Location Bar will stop updating the text and entering
new links won't work.

Before someone marks this 'worksforme' or 'invalid', I'll explain what I did to
make it work again, and the steps surrounding what happens when it fails.

Steps to Reproduce:

1.  I had launched the app with composer simultaneously, and both started at the
same time.
2.  The browser loaded http://home.netscape.com
3.  Then, I typed http://www.news.com into the browser and hit enter.

Expected Results:

The previous url is replaced, and the new website loads.

Actual Results:

Things that *worked*:

Bookmarks would load, but not replace the URL field.
Personal toolbar links would also load, but not replace the url field.
Doing a CTRL-N, which spawned a new window, worked - everything worked fine in
this window. 

Things that *didn't work*:

Dragging the Page Proxy icon to the personal toolbar still had
http://home.netscape.com as a link, instead of the current URL.
I use Windows XP and Mozilla 1.0 RC3, and have noticed this bug too. It is not
happen very often, but sometimes when I have open 3 og more mozilla windows, and
click the mozilla shortcut on my desktop, the location bar will stop working. I
can't type anything in it. The only thing that work is the [Delete] key, to
clear the location bar.
I've run into this on Win98, build 2002053012.

What did work:
  * Clicking the throbber took me to Moz.org; the favicon changed to Mozilla's,
      even though the URL was not displayed.
  * Selecting a URL from the "Recent" list.
  * The "Go:" button.
  * The "Open Location" dialog.
  * Bookmarks.

What didn't work:
  * Typed text.  It appeared as typed, but nothing happened when I pressed
      enter.
  * Opening a new window.
  * Closing all windows and restarting Mozilla.  (I do not use the launcher.)
  * Back and Forward buttons were grayed out the whole time.

Mozilla was working properly for a few pages, then spontaneously misbehaved.
Stephend: Do you see this with Mozilla 1.1 or later ?
Not lately, but this isn't a 'I can reproduce at will' kind of bug, either.

I've observed something similar ever since I installed 1.4a, on both Win2K and
OS X.  The location bar never updates the entries, making it necessary to
re-type my frequently-used URLs every time I wish to access them.

My location bar preferences are set thus on all my machines:

X Show list of matching results
X Match only websites you've typed previously

(The other two options are unchecked.)
Using WinXP and Moz 1.4rc2

Any URLs typed in the location bar correctly send the page to the URL, but it's
not really *in* the location bar. The proxy icon is grayed out and not
draggable. And links (on the page, via bookmark, via personal toolbar) clicked
do not update the location bar with the new URL.

Same text stays in URL across any open tabs.

I have a Bookmark set up to send all URLs in the form of "? myqueryhere" to be
sent to Google. This does NOT work anymore -> I'm sent to Netscape's search page
(even though in my preferences Google is my chosen search page).

Back button does NOT work (grayed out), but history entries are created (I can
see my correct and up to date browsing history with Ctrl+H). Using the Backspace
key lets me perform the action of the Back button, but Alt+LeftArrow does NOT.

Any ideas? I believe that this just started happening since the 1.4rc2 install.
I’ve started seeing this in the last few revisions of Firefox. Mozilla/5.0 (Macintosh; U; PPC Mac OS X Mach-O; en-US; rv:1.8.1.11) Gecko/20071127 Firefox/2.0.0.11

Sometimes I open a new window, the location bar and the back/forward buttons stop working. Every other part works fine. The favicon is updated. The history menu gets longer and javascript:history.back() works just fine. It just appears that there is a disconnect between the navigation toolbar and the content. New tabs in a working window work, but when it breaks, all new windows are broken.
I have the same problem on Firefox too under Ubuntu:

Trying to use the box that opens when you press ctrl+l with the navigation bar turned off doesn't work either, but the Google search box and such all work fine.

For me the location bar continues to update it just won't accept any new sites with either pressing enter or clicking on green triangle.

I think it was also happening in Firefox3 beta 2.
I'm having this same problem on OS X 10.5.3 in Firefox3 beta 2.

I can't seem to go to any URLs that are entered into the location bar and clicking on the history links that pop up doesn't take me to the pages either.  I have to restart the browser in order to get this functionality back.

Also seeing this in Firefox 3.0.0 on Win2k.  I'd seen this behavior a long time ago in past versions and then it disappeared.  It was even absent in the RCs for FF3, as far as I could tell.

I cannot figure out how to reproduce this bug, but I will keep watch.  While signing up for a Bugzilla account I noticed that the search box has also stopped working: I can enter a search term but pressing Enter does not load the search page.
I'm seeing this in Firefox 3 on Mac OS X 10.5.3 Like Micah, I've seen this behavior come and go through several builds, but its back again. Twitter responses indicate I'm not the only one...
I should add that I'm not seeing the problem with text not updating - mostly just the problem with entering links. I can type, but it won't go to that page with a carriage return or clicking on "go" until I restart Firefox.
