This is a required feature as per the 'specs' and still has yet to be implemented. Granted it won't work right until bug 7830 is fixed but it still needs to be done. As far as I can tell there should be a button on the Personal Toolbar labled 'Bookmarks' with a folder icon. Clicking it should produce a popup (menu?) showing the hierarchy of the stored bookmarks.

Any scope for in-place bookmark management here? For example, on the Win 98 start menu, you can right click on an item and remove it. I would say also allowing dragging them around on a menu, but that feature usually annoys me more than I want it (perhaps enable it on right mouse button drag?).
In a word, yes. It is my understanding that sort of functionality will be included.
