Introduce a new extensions toolbar button (widget)

• shown when pref is set, if there are any extensions enabled
• always in every toolbar, can't be customized away
• clicking it opens a panel with a list of all (visible) extensions
• each row is an icon, an extension name and a >> submenu button
• below all is a [Manage Extensions] item that opens about:addons/extensions
