As the Home and Refresh buttons were recently changed, the preview icon and image when you go to Add-ons --> Themes and view "Firefox (default)" need to be updated to match the new icons. There could be other changes to the icons that need to be updated here, too, but those are the two things that came to mind. :)

It looks to me like this is fixed now. CVS says it was 345466. Dupe?
Not fixed yet, the home icon is wrong (and that's likely to change again)
