If you create an new e-mail message using the 'Create a new message' toolbar button, the 'Insert' menu option is available.

However, if you use the 'Edit as new' feature on a previously sent e-mail message, the 'Insert' menu option is not available.

Updated 'Component' classification to "Message Compose Window".
Component: Mail Window Front End → Message Compose Window
Insert is only available when composing in html mode, if the mail edited as new is plain text, composition will open in plain text mode and insert it not available as there's nothing to insert in a plain text msg.
