In the new message header the reply list button has an empty icon since a couple of weeks. Also when you add the reply to list button to the toolbar, you end up with an empty icon next to your label... This also happens in Safe Mode.

On the toolbar, the icon is only missing when you use small icons. Normal icon is showing up normally...

This was because of the human factor, also known as andreasn. :(
I must have missed this when assembling together the imagemap for bug 524443. The patch is rather simple, so do we need a ui-review here?
