What did you do?

Create an new page

What happened?

The block "Review needed?" is missing, therefor it's not possible to ask for technical nor editorial review when you creating a page. 

What should have happened?
==========================
The block "Review needed?" should be there with the technical review and the editorial review checkboxes already checked.

Is there anything else we should know?
======================================
I've had this happen a couple of times, but I'm not sure what circumstances brought it about.
