Bug 674927 made it so that spelling could be disabled in contenteditable areas in which case we should hide the spelling UI for those areas.

Because of bug 906979 I have also disabled the spelling items for inputs and textareas with disabled spelling, but this could be backed out as necessary.
There is a testcase for disabled spelling in contenteditable in bug 905176.
