969519 - Track user dropoff rates through the feedback collection form

We're curious to know how many people dropoff between wanting to give feedback and actually clicking submit. We may be adding more features to the feedback flow and this is a good strong metric to make sure we make things better and don't get in people's way.

I redid the forms, so it's easier to do drop-off measuring now.

One thing to note is that the two forms that need measuring are the generic feedback form and the Firefox OS form. We want to keep measurements separate. I.e. card 1 for the generic form is not the same as card 1 for the Firefox OS form.

I call the "ua-team" the "analyzers" group. So tweaking the metadata accordingly.

That landed in master in https://github.com/mozilla/fjord/commit/3b092e4650011912098aea9cda819646a1068ed0

That was pushed out to production by me just now.
