STR:
- check the box "Adverse Event" in "Report Type"
- set Month to 12, Day to 13 and Year to 14 in "Date of this report"
- save the pdf
There are no issue when opening the saved pdf in Firefox or evince or mupdf.
But in masterpdf or Chrome, the Day textfield is empty and it becomes a button (??!!).
