Original GH issue tracking requirements: https://github.com/mozilla/pdf.js/issues/7613
Original proposal also describing spec: https://docs.google.com/document/d/1lKa9aMEJ5Nbg9jn-KN6c1FrY4iDIaxHOLNPwk0mFqtw/edit#
Note, both of these are out of date with regards to current status. Should we close or update them?

Form saving is still buggy. This form is used by our local court system for billing:
https://www.mass.gov/doc/guardian-ad-litems-certificate-of-servicesitemization-of-services-jv-068/download
If you fill it out in Firefox and save, the resulting PDF is scrambled. The data entered into the fields goes into the wrong place. A name field on page 2 gets saved on top of a different one (the first one), which is lost. When page 3 is filled out, it is overlaid on top of page 2, though if page 3 is not full, the bottom of page 2 is still unchanged, but page 3 is blank. It's just a terrible mess.

Thanks Fred for reporting this issue. Could you open a bug for it?
