Insert the in-app-notification-manager into the application when the preference is enabled.

Acceptance Criteria:

• The manager should be inserted into the application
• The Dom position for the manager should be stable to keep notifications in the correct place in the tab order.
• The templates for the various in-app-notification components should be included
• The manager should be included on startup only if the pref mail.inappnotifications.enabled is true
• the manager should be added or removed if the mail.inappnotifications.enabled preference is changed
