We have two feedback forms: desktop and mobile. They differ slightly in language and only the desktop one has a url field. This has a maintenance cost that has no obvious benefit. Further, we're missing out on url information from the mobile case. I think this bug should cover the following work: 1. unify the mobile and desktop feedback forms into a single form 2. fix the privacy-related informational text (see bug #946440) 3. fix strings so that the product name isn't hard-coded--it should come from the template variables
Putting this in the 2014q1 queue. Fixing this makes it easier to generically support multiple products, so it should get done.

I think I want to tackle this asap because it's way easier to maintain one form than two. Turning this into a tracker and spinning off a bug for merging the two forms without changing strings and a bug for fixing the form so it uses the product string.
