Refine card layout for the medium sized card, in the case where excerpts are not included for Ads and Organic content

- We must keep medium cards to >250px for IAB ads

- We will make this change available anywhere the feed is available

This change will be behind a feature flag, and will not go live until we get sign-off from Ads. We may need to update our sales pipeline to exclude the excerpts before we can turn this on for all users
