Cleaning this up. Sorry for the confusion. From what I can tell, we have two site permissions app bars, the location one and the offline storage one. I'm moving the remaining location app bar dependency to block the location-specific app bar story at bug 845155. I've moved the remaining offline storage dependencies to block the offline-specific app bar story at bug 849396.

I think we can close this story out as fixed (as much as this one is going to get.) What's left to do is tracked in individual site permission app bars rather than this generic bug.

OK. This story has been updated to cover simply showing a working version of the site permissions request info bars. Specific details about those app bars continue in their own stories.

Tested on 2013-03-12 on nightly built from http://hg.mozilla.org/mozilla-central/rev/7433bc4545c9 - Verifying using links in main success scenarios for geolocation and offline storage permissions, as described in the attachment in comment #6 - Verified that the different options in the info bar for both types of locations worked. - There is no way to toggle the "never for this site" preference which is similar to the problem with the password info bar, but perhaps less common.
